Applied for trainer's license on 6/30/07 at Tucson; was denied on 7/11/07 due to past violations; on 7/17/07, he appealed & Director held hearing on 7/19/07 - license granted as long as Mathieu submits to fingerprint & background check, agrees to a probationary period of 1 year from date of issue, continues to seek medical assistance and take his prescribed medications, refrains from consuming alcohol on property & must submit to random alcohol/drug screenings & must provide the name of another licensee willing to act as a personal mentor to Mathieu |
